Required Documents for Foreign Travelers
Documentation requirements for foreign travel are listed below.
Those wishing to have advance payments done on their behalf:
- A faxed copy of the completed and signed Certification of Academic Activity (T182-27) form
- If Traveler is a Resident Alien; a copy of their "Green Card"
- If applying for a J1, a copy of the application from International Services
For International Visitors wishing to be reimbursed for Travel Expenses, including citizens of Canada & Mexico:
- Copy of traveler's passport photo page
- Copy of the traveler's Visa (in their passport) OR a copy of the ESTA Approval form (if traveler entered the U.S. on the Visa Waiver Program)
- Copy of the Passport Stamp in the traveler's passport, showing when they entered the U.S. OR a copy of their I-94. Travelers can obtain their I-94 from the U.S. Customs and Border Protection website.
- Copy of the completed and signed Certification of Academic Activity (T182-27) form (for travelers who entered the U.S. on a B-1, B-2, WT, or WB visa, including the Visa Waiver Program/ESTA)
- Requests for reimbursement as a Foreign wire transfer requires a completed typed and signed Wire Transfer Request form
All required documents must be uploaded on the "Image Scanning" tab of the Travel Reimbursement eDoc. Please ensure any Passport Numbers and/or Social Security Numbers are redacted prior to uploading to scanned images.
Those entering the U.S. while in the process of applying for Permanent Residency:
- US Social Security number
- Copy of passport with visa stamp - will indicate application status for Permanent Residency with a number stamp beginning with an 'A' - No I-94 will be given to these visitors.
- Completed Form 1078 - Certificate of Alien Claiming Residence in the US
